About Link
Link Rehab & Performance brings clinical care, wellness, fitness, and performance services together under one organization. Depending on what you need, our team can support in-home rehabilitation, palliative care, virtual services, weight management, personal training, group fitness, and corporate wellness.
Our goal is to understand where you are now, where you want to go, and help connect you with the right level of support.
No. Every Link service can stand on its own.
Some clients come to Link specifically for rehabilitation. Others may only want personal training, weight-management support, group fitness, or a corporate wellness program.
The advantage of the Link model is that additional services may be available if your needs or goals change over time.
Link is built around individualized service rather than placing everyone into the same system. For applicable programs, the professional comes directly to you.
Our clinical and performance backgrounds also allow us to consider movement, function, strength, fitness, health, and long-term goals together rather than viewing each area in isolation.
Link serves a broad range of clients. Depending on the service, this may include older adults, people recovering from surgery or changes in mobility, individuals living with complex health needs, busy professionals, adults pursuing weight-management goals, people returning to exercise, and athletes looking to improve strength and performance.
The appropriate program depends on the individual and their goals, not simply their age.
Yes. Your needs at the beginning of a program may not be the same several months later.
Link is designed around progression. When appropriate, the plan can be adjusted and our team can help identify a logical next step.

Getting Started & Scheduling
Start by completing the Get Started form or contacting Link directly.
We'll ask a few basic questions about the service you're interested in, your location, your goals, and how you prefer to be contacted. From there, our team can help determine the appropriate next step.
A member of the Link team reviews your inquiry and contacts you using the method you selected.
We'll learn more about what you're looking for, confirm current service availability in your area, and explain the next step for the program you're interested in.
Absolutely. You do not need to diagnose your own needs or know exactly which Link program is right for you before contacting us.
Tell us what's going on and what you're hoping to accomplish. We can help point you toward the most appropriate place to start.
Yes. Family members and caregivers are welcome to contact Link to learn more about available services.
When clinical care is involved, appropriate patient consent and required documentation will still need to be addressed before care begins.
Availability depends on the service, location, professional availability, and any insurance, referral, or authorization requirements that may apply.
Once we understand what you're looking for, our team can explain the appropriate next steps and current availability.
Yes. We can take scheduling preferences into consideration and will work with you based on professional availability in your service area.
Contact Link as soon as possible if your plans change. Cancellation and rescheduling policies may differ by program and will be reviewed with you before services begin.

In-Home Rehabilitation & Clinical Care
In-home rehabilitation brings skilled therapy into the environment where you actually live. Depending on your needs, services may include physical therapy and occupational therapy focused on mobility, function, independence, safety, and meaningful everyday goals.
Depending on individual needs and eligibility, in-home rehabilitation may be appropriate for people recovering from orthopedic surgery, hospitalization, neurological conditions, changes in balance or strength, reduced mobility, deconditioning, or difficulty performing everyday activities.
The treatment plan is individualized following an appropriate evaluation.
Physical therapy often focuses on areas such as mobility, walking, balance, strength, transfers, and physical function.
Occupational therapy focuses on helping people participate safely and independently in meaningful everyday activities such as dressing, bathing, household tasks, routines, and community participation.
Some clients may benefit from one discipline, while others may benefit from both.
You may contact Link directly to discuss therapy services.
Referral, authorization, documentation, or insurance requirements can vary depending on the service and payer. Our team can help explain what is needed before care begins.
The first visit generally focuses on understanding your current abilities, concerns, home environment, relevant history, functional challenges, and personal goals.
Your clinician can then establish an appropriate plan of care and explain what treatment will focus on.
Link aims to provide continuity whenever possible because familiarity and consistency can support a better care experience.
Scheduling, clinical needs, or availability may occasionally require another qualified professional to participate in your care.
Usually not.
Your clinician can evaluate what you already have and determine whether any equipment, mobility device, safety solution, or environmental modification may be helpful.
Yes, when appropriate. A home safety assessment may consider mobility, transfers, bathroom safety, stairs, fall risks, everyday activities, and equipment needs within the environment where the client actually lives.
Potentially, yes. Post-operative rehabilitation is one of the situations in which in-home therapy may be appropriate.
The exact plan depends on the individual's procedure, precautions, functional status, medical recommendations when applicable, and payer requirements.
No. Depending on the service and circumstances, care may be provided in appropriate residential environments such as apartments, condominiums, and certain senior or residential communities.
Availability can depend on facility policies and the specific service.

Palliative Care

No.
Palliative care can be provided alongside ongoing medical treatment and is not limited to end-of-life care.
Link Rehab & Performance currently offers palliative-care-related services; hospice is not listed as a Link service.
Someone living with a serious, progressive, or complex illness may benefit when symptoms, functional challenges, daily activities, caregiver demands, or quality of life become increasingly difficult to manage.
Eligibility and the appropriate services depend on the individual's circumstances.
Depending on clinical need and the individual care plan, rehabilitation may support mobility, safety, energy conservation, independence, comfort, and participation in meaningful daily activities.
Yes, when appropriate and authorized by the patient. Education and caregiver support may be incorporated when they are relevant to the individual's care and goals.

Teletherapy, DME & Home Safety
Depending on the service and individual situation, virtual support may be appropriate for education, follow-up, caregiver training, certain therapy-related needs, equipment guidance, and other situations that do not always require an in-person visit.
No. Some needs require an in-person or hands-on evaluation.
If virtual care is not appropriate for your concern, our team can explain other available options.
Generally, you will need an internet-connected smartphone, tablet, or computer with a camera and microphone.
Any specific technology requirements can be reviewed before the appointment.
Yes, when appropriate and with the patient's permission.
This may be particularly useful when a caregiver is involved in mobility, equipment use, home safety, or everyday routines.
Link may help evaluate and guide appropriate equipment needs within the scope of the applicable service.
Examples may include mobility and safety equipment such as walkers, rollators, wheelchairs, bathroom equipment, and other appropriate devices.
Yes, when appropriate. A wheelchair assessment may consider mobility, positioning, safety, environment, functional needs, and the features required for an appropriate mobility solution.
Equipment coordination can depend on the device, clinical need, supplier, documentation requirements, and insurance or payment arrangements.
Our team can explain the process that applies to the equipment you need.
Some education and follow-up may be appropriate virtually, but certain equipment and safety decisions may require a more detailed assessment of the individual and their environment.

Weight Management
Link's programs are built around professional guidance, accountability, measurable progress, and a structured plan rather than simply giving someone a calorie tracker or generic program.
Your starting point, goals, lifestyle, activity level, and progress help shape the experience.
Link's primary weight-management programs are structured around a 12-week commitment, providing time to establish routines, measure progress, build accountability, and make meaningful adjustments.
No.
LINK Foundations focuses on coaching and accountability without weekly in-home training.
LINK Momentum combines the weight-management structure with in-home private training.
LINK Performance provides a higher-touch level of support with roughly twice-weekly private training.
Link's current weight-management programs focus on structured coaching, accountability, fitness, sustainable habits, and lifestyle change. They are not medication-based medical weight loss, and Link does not prescribe weight-loss medication.
The goal is not to give every client the same restrictive diet.
The program is designed around sustainable habits, accountability, activity, fitness, and realistic changes that can be maintained beyond the initial 12-week program.
Progress can involve more than the number on a scale.
Depending on the program and goals, progress may include weight trends, activity consistency, strength, endurance, body measurements when appropriate, fitness performance, habit adherence, and other agreed-upon goals.
At the end of the initial program, progress can be reviewed and the next step discussed.
Depending on your goals, this may include continued accountability, additional training, maintenance support, or transitioning into a longer-term fitness program.
No.
Some clients may have significant weight-loss goals, while others may be focused on improving body composition, consistency, fitness, strength, energy, or overall health behaviors.

The purpose of the initial process is to understand your starting point and build a realistic program from there.
